New Guidelines for Applying for the Unified National ID card
The Consulate General of the Republic of Iraq in Detroit invites citizens who wish to apply for the Unified National Card to follow the new procedure issued by the relevant Iraqi authorities. This includes booking an appointment online in advance before coming to the consulate building to submit the required original documents, as shown below, without sending it through email.

Required Documents When Visiting the Consulate
1- Complete the National ID application form via the link below (must be printed on a single page):

2- Original Iraqi Certificate of Nationality and original Civil Status ID.
3- One recent personal photograph.
4- A valid passport (Iraqi or U.S.).
5- A recent Iraqi nationality certificate, Unified National ID card, or electronic record extract of the father, brother, or sister.
New Birth Registration
A birth certificate issued outside Iraq must be authenticated by the Iraqi Ministry of Foreign Affairs and the Iraqi Ministry of Health using the barcode system. After that, an appointment must be booked online, and the applicant must attend the Consulate to complete the procedures. It is required to bring the father’s Unified National ID card (if available) or a recent Iraqi nationality certificate.
Notes
1- In case of loss of the Civil Status ID, Iraqi Certificate of Nationality, or both, or the National ID card, a police report must be provided, translated into Arabic, and certified by the Secretary of State.
2- If the National ID card has expired, an appointment can be booked for renewal.
3- If the applicant for the Unified National ID card lacks mental capacity, a guardianship document issued by Iraq or the relevant U.S. authorities must be submitted, translated into Arabic, and certified by the Secretary of State.
