National Card
The Consulate General of the Republic of Iraq in Detroit is pleased to announce the launch of the Unified National Card System and the acceptance of applications from those wishing to obtain a National Card. To organize your work, please follow the following steps:
- Fill out the National Card application form via the link below and save it in PDF format or print it after downloading and installing the program.

- Scan your original Iraqi citizenship certificate and civil status ID and save them in PDF format.

- Follow the website of the Consulate General of the Republic of Iraq in Detroit and its Facebook page to view the appointment schedules for applicants whose applications are approved from Baghdad.
- The national card fee is 6,000 Iraqi dinars (six thousand Iraqi dinars). Payment must be made by credit card (MasterCard or Visa). Cash payments are not permitted.
Please see the instructions below regarding the issuance of the unified national card.
First:
The unified card application form is not required in the following cases:
Electronic record holders (those with an electronic record image).
Expired national card.
Damaged national card.
Lost national card.
Second:
In the event of loss of any documents (civil status ID, citizenship certificate, or national card), the citizen must obtain a lost report from the police station in his area of residence. This report must be translated into Arabic, authenticated by the Secretary of State, and certified by the consulate.
Third:
If you wish to register a marriage, you must bring a marriage contract issued in Iraq or a marriage decree issued by an Iraqi court. This also applies to registering a divorce.
Fourth:
If you wish to register births, you must bring a birth certificate issued in Iraq or a copy of a birth certificate issued in Iraq addressed to the Civil Status Department. This also applies to registering deaths.
