Retirement Life Certificate
A retirement life certificate is issued according to the regulations listed below:
The retiree must appear in person and complete the life certificate form.
The original and a copy of the civil status ID, Iraqi nationality certificate, unified ID card, or Iraqi passport.
A copy of the retirement ID.
Two recent personal photos with a white background (size 2×2).
A fee of $20 (cash only).

(Life Certificate and Affidavit):
Requirements for issuing a life certificate and written declaration for all types of powers of attorney must be submitted in person at the consulate building. The effective date must be at the end of each year.
A copy of the power of attorney (of attorney) (of any type) for which a life certificate and written declaration are to be issued, in color, along with the original civil status ID and a color copy thereof, the original Iraqi citizenship certificate and a color copy thereof, or a copy of the electronic copy of the national ID number, or a copy of the national ID card.
A color copy of the Iraqi document of the agent.
Two (2) recent personal photos (“2 x 2” with a white background), issued no more than six months ago.
$20 (twenty US dollars) (cash only).
(Certificate of Regular Life):
Requirements for issuing a Certificate of Regular Life must be submitted in person at the consulate building. The effective date must be at the end of each year.
Original civil status ID and a color copy, or an original Iraqi citizenship certificate and a color copy, or an original, valid Iraqi passport and a color copy.
Two (2) recent passport-size photographs (“2 x 2” with a white background), issued no more than six months ago.
$20 (twenty US dollars) (cash only).

(Mail Retirement Life Certificate):
We would like to draw the attention of the esteemed community regarding the issuance of a retirement life certificate (exclusively) by mail. To ease the travel burden for retirees, please follow the steps below.
Retirees are required to send a self-addressed envelope to the consulate address (16445 W 12 Mile RD. Southfield, MI, 48076)
containing:
A copy of (civil status ID, Iraqi citizenship certificate, unified national ID card, or Iraqi passport, a copy of the retirement ID, and a copy of the American ID).
A copy of a previous life certificate, if available.
Two (2) recent personal photos.
The fee for issuing a life certificate is $20 (cash only).
If applying for the first time or if a copy of a previous retirement life certificate is not available, a special form for a retirement life certificate must be completed.
A prepaid, registered mail return envelope containing the retiree’s address, for return after the official life certificate form has been printed by the consulate.
After the retiree receives the return envelope containing the official life certificate form prepared by the consulate, the consulate will video call the retiree during official working hours to verify the full name, signature, and fingerprint in the designated spaces on the official form. (Please provide fingerprints and a black or blue pen.)
The retiree will then send an envelope containing (2) of the official forms signed by him/her, along with a prepaid, registered mail return envelope containing the retiree’s address, for the purpose of sending the retiree’s copy after it has been stamped and issued by the consulate.
